How to send Forms to Patients Using Doctible's Dashboard

1. Load Dashboard
2. Click "Send Form to Client"

3. Click "Intake Form" drop-down and select your form

4. Click the "Name" Field and enter patient's name

5. Click "Email" Field and enter patient's contact details

6. Click "Send Form"

How to Send Forms to Patients Using the Form Templates Library

1. Go to Dashboard and click "Manage Templates"

2. Select Form Click the ("...") icon

3.  Click "Send to Client" From drop-down

4.  Click "Name" and "Email" Fields. Write client's details.
